HEALTH REQUIREMENTS

REQUISITOS DE SALUD

The Marquardt District 15 Department of Health Services is dedicated to the promotion and maintenance of the health and wellness of the school community. The role of the health services staff is to promote the physical, mental, emotional and social well being of students with the goal of helping them achieve optimal learning.

Health Requirements by Grade

Pre-K

Complete Physical Exam and all required immunizations (see below)

Exam form: State of Illinois Certificate of Child Health Exam

State of Illinois Certificate of Child Health Exam_ Spanish

D.P.T. series: Three doses by age 1, plus an additional dose by age 2
Polio series: Two doses by age 1, and an additional dose by age 2
MMR (measles, mumps, rubella) vaccine: One dose on or after age 1
Varicella (chickenpox) vaccine: One dose on or after age 1, or proof of varicella disease or immunity
Hepatitis B (HBV) series: Three doses required. Third dose must have been administered on or after 6 months of age
Haemophilus Influenza Type b series (Hib) vaccine: One dose given after age 15 months
Pneumococcal conjugate series vaccine: Primary series complete, or 1 dose on/after 24 months of age

Kindergarten

Physical and eye exam due prior to October 15th of the current school year

Dental exam due prior to May 15th of the current school year

Complete Physical Exam and all required immunizations (see below)

D.P.T. series: Four or more doses, with the last dose qualifying as a booster and received on or after the fourth birthday
Polio series: Four or more doses of the same type of Polio vaccine, with the last dose qualifying as a booster and received on or after the fourth birthday
MMR (measles, mumps, rubella) vaccine: Two doses, with first dose on or after the first birthday and second dose no less than four weeks later
Varicella (chickenpox) vaccine: Two doses, with the first dose on or after the first birthday and second dose no less than four weeks later, or proof of disease or immunity
Hepatitis B (HBV) series: Three doses recommended. The third dose must have been administered on or after 6 months of age

6th Grade

Physical due prior to 10/15/2023

Dental exam due prior to 5/15/2024

Optional sports physical due prior to tryouts or any participation in interscholastic sports

Tdap booster: Upon entering sixth grade

MMR (measles, mumps, rubella) vaccine: Two doses, with the first dose on or after the first birthday and the second dose no less than four weeks later
Varicella (chickenpox) vaccine: Two doses, with the first dose on or after the first birthday and the second dose no less than four weeks later, or proof of disease or immunity
Meningococcal Vaccine: One dose on or after the 11th birthday

Complete 6th grade dental exam: Dental Examination Form

Optional: A sports physical must be turned in prior to participation in any interscholastic sporting event including tryouts

Parent and Guardians:

To report a student absence, press 7, to reach a nurse press 9. If we do not receive your student's absence, a school staff member will contact you.

If you need to change the way your student is going home, you must call the office by 2:00pm to assure communication. Students cannot be pulled out of line or off the bus at dismissal due to safety reasons.

Please note: Students who leave school due to illness will not be allowed to participate in after school activities.

FAQ: When should I keep my child home from school?

WHEN TO KEEP YOUR CHILD HOME FROM SCHOOL:
- When student has had a fever of 100° or more within the last 24 hours.
- When student has thrown up or had diarrhea within the last 24 hours.
- When student has a rash that has not been seen by a doctor.
- When student has pinkeye that has not been treated for a minimum of 24 hours.
- As instructed by the local health department related to communicable (including COVID-19) and chronic disease guidelines. Parent/Guardian will need to bring in note stating student is able to return to school.
